Steps:

2.A form will appear Like this ... You need to fill the form given there...


                               

3.Just Enter Your Surname and Date Of Birth and Capatcha.... No need to enter first name and middle name,,,,,

4.After entering all the details in the form.. Click on Submit.. If the information was given is correct then it directs to a new page showing your Pan card information...............
